The Core: Three Actions on Every Referral

This is the heart of the system. When a case manager submits a referral form, three things fire at once:

  1. Their outreach opportunity flips to Referral Submitted.
  2. A new referral pipeline opportunity is created, sitting in Under Review with every detail from the form already populated on the card.
  3. The same person is logged as an active partner in a separate pipeline.

Because matching the right caregiver to the right family genuinely matters, referrals are fully filterable by service, detail, city, race, ethnicity, and gender. The team is not scrolling through fifty cards to find the one that fits.
